My eldest just hit 12 - must make a new IC for her - Standard Form to fill

In case anyone out there is also looking at making a new IC for his or her child that has just turned 12, this is the form that you need to fill in before you get to the JPN counters. Bring along with you (originals) birth certificate (and MyKid, if possible) as well as the IC of one of the parents.

Do it before 30 days pass the kid's birthday. Otherwise you will be fined.
